1. What are the terms and conditions of the OBOZ program?
To be accepted into the program and receive the Zaky for a hospital::
The Zaky must be shipped to a medical professional of the NICU. Zakeez Inc. will request
authorization to the Nurse Manager or Medical Director of the Unit prior to processing
It must be used in the NICU and according to the instructions
The Zaky will not be sold or charged to the patient
Only one donated Zaky per hospital will be approved in the program. Please visit our Hospital Network to see which hospitals are already in the program, so they are not elegible to receive more Zakys. If you don´t find the hospital in this list, it is elegible.
Zakeez, Inc. reserves the right to deny any application and the right to change or cancel the program at any time

2. Can anyone request the Zaky for NICUs and how much it costs?
Any adult can pay the minimal admin cost ($7.98) plus the shipping and handling fee and after the payment process you will fill a form with the information of the hospital. Once approved, the Zaky will be shipped within one week.
3. Can I request the Zaky on Behalf/Memory of someone else?
Yes. We will donate the Zaky and we can include a hand-made card with a personalized note from you.
4. Will the hospital know my information if I request the Zaky for them?
Only if you indicate it. You may remain anonymous, or we can send your name, address and/or phone number with a greeting card. We will only share the information that you approve.
