1. What are the terms and conditions of the OBOZ program?
To be accepted into the program and receive the matching Zakys for a hospital:
After completing the order, you will be redirected to a form about the hospital information that must be filled and sent online
The Zaky must be shipped to a medical professional of the hospital (NICU, PICU, Labor and Delivery), etc.. Zakeez Inc. will request
authorization to the Nurse Manager or Medical Director of the Unit prior to processing
It must be used in the hospital and according to the instructions
The hospital cannot sell or charge The Zaky to the patients
Zakeez, Inc. reserves the right to deny any application and the right to change or cancel the program at any time

2. Can anyone request the donation of Zakys for hospitals, and how much it costs?
Any adult can but at least 3 Zakys at regular price ($49.95 each) and we will match the donation one-to-one.

3. Can I donate the Zakys on Behalf/Memory of my loved one?
Yes. We can include a hand-made card with your personalized greeting. Please indicate it in the form after the order is complete.
4. Will the hospital know my information?
Only if you indicate it. You may remain anonymous, or we can send your name, address and/or phone number with a greeting card. We will only share the information that you approve.
